TERMO DE ORIENTAÇÕES

A Maison Depil – Unidade PAULISTA, utiliza o produto Depimel do Brasil LTDA, com registro na ANVISA nº 343/05.

1. CASOS EM QUE A DEPILAÇÃO NÃO DEVE SER REALIZADA (cabendo ao cliente avisar ao estabelecimento sobre quaisquer das situações abaixo):

– Alergia à cera de abelha,
– Gestante (sem autorização do médico),
– Uso de qualquer tipo de medicamento para tratamento de pele como ROACUTAN ou similar que por si só já provoque reações adversas,
– Uso de Ácido Retinóico na área depilada,
– Apresente algum problema de pele,
– Dermatites,
– Tratamento dermatológico com reações ou recomendação médica contrária à depilação,
– Pele bronzeada ou avermelhada pela exposição ao sol,
– Tiver apresentado reações adversas em depilações anteriores.

2. CUIDADOS APÓS A DEPILAÇÃO

-Usar protetor solar na área depilada,
– Não se expor ao sol por no mínimo 2 dias.

3. REAÇÕES POSSÍVEIS APÓS A DEPILAÇÃO:

– Foliculite,
– Avermelhamento da pele,
– Irritação na área depilada,
– Pelo

Declaro ter recebido as instruções acima, nos termos do artigo 6º do Código de Defesa do Consumidor. Declaro não me enquadrar nas situações indicadas no item 1, autorizando a realização do procedimento, assinando o documento no campo abaixo (da página 2 em anexo).

 

INSTRUCTIONS DOCUMENT

Maison Depil – PAULISTA, uses the product Depimel do Brasil LTDA, registered with ANVISA under number 343/05.

1. CASES IN WHICH DEPILATION SHOULD NOT BE PERFORMED (the client must inform the establishment of any of the following situations):

– Allergy to beeswax;
– Pregnancy (without medical authorization);
– Use of any type of medication for skin treatment such as ROACUTAN or similar that may cause adverse reactions;
– Use of Retinoid Acid in the hair removed área;
– Present any skin problem;
– Dermatitis;
– Dermatological treatment with reactions or medical recommendation contrary to depilation;
– Tanned or reddened skin due to sun exposure;
– Have experienced adverse reactions in previous depilations.

2. CARE AFTER DEPILATION

– Use sunscreen on the depilated área;
– Avoid sun exposure for at least 2 days.

3. POSSIBLE REACTIONS AFTER DEPILATION:

– Folliculitis;
– Redness of the skin;
– Irritation in the depilated área;
– Ingrown hairs.
